Per Diem LVN
Position Summary
Under the direction of a Registered Nurse, the Hospice Licensed Vocational/Practical Nurse provides skilled nursing and palliative care for assigned hospice patients. He/she works cooperatively with the other members of the Interdisciplinary Hospice Team. The Hospice LVN provides intermittent visits or shifts of skilled nursing care, performs prescribed medical treatments, including pain management and symptom control, provides nursing observations, provides teaching regarding the expected course of the disease, care techniques and other health measures to the patient, family or other primary care giver, provides counseling and supportive care to the patient and family, and participates in the care plan development and implementation.
Qualifications
- High school Diploma or GED.
- Graduate of an accredited school of vocational/practical nursing.
- Current license within the State. Certification in Hospice and Palliative Vocational/Practical Nursing.
- At least one year experience as a LVN/LPN.
- Demonstrated competency in performance of skilled nursing functions.
- Ability to work with dying patients and their families. Possesses a sympathetic attitude toward caring for the hospice patient and demonstrates positive communication skills in interacting with other members of the hospice team.
- Valid California driver's license and auto insurance.
- Ability to maintain a high level of confidentiality concerning patient and family information.

- Report changes in the patient’s conditions and needs to the RN Case Manager or Director of Patient Care Services.
- Administers prescribed treatments and medications as permitted by scope of practice.
- Accurately describe patient problems in relations to diagnosis.
- Provide those services requiring nursing skills in accordance with the plan of care.
- Educate and instruct the patient and patient's family.
- Demonstrate flexibility in patient scheduling and readily accepts new assignments.
- Observe for safety hazards in the patient’s and organization’s environments and takes appropriate steps to reduce or eliminate them.
- Maintain continuing education through academic studies, seminars, and workshops.
- Attend and participate in in-services, staff meetings and case conferences.
- Complete all documentation in an accurate, legible manner and in the timeframe required by standards and regulations.

While performing the duties of this job, the employee is occasionally exposed to moving mechanical parts. The employee will be required to operate standard equipment used by registered nurse in performance of duties: blood pressure cuff, pain reduction pumps, drawing blood, etc. The noise level in the work environment is usually moderate to occasional low.
