QAPI RN
Position Summary
The QAPI RN has the responsibilities for the coordination and implementation of the QAPI program. He/she reviews hospice medical records for assessment of the quality of clinical services to ensure adherence to all policies and procedures and applicable standards and makes recommendations for changes in policies, procedures, systems and for staff education.
Qualifications
- Graduate of an accredited school of nursing or school of vocational/practical nursing.
- Current license within the State as an RN or LVN/LPN.
- At least two years of varied work experience as a licensed nurse and preferably one of the two years as QAPI nurse. One-year hospice experience.
- Documented QAPI training.
- Ability to make meaningful observations and write comprehensive accurate reports
- Knowledge of QAPI functions/Medicare COPs, State regulations.
- Excellent communication and organizational skills and detail orientated.
- Possesses a sympathetic attitude toward caring for the hospice patient and demonstrates positive communication skills in interacting with other members of the hospice team.
- Proficient in Microsoft office applications and Misys.
- Valid California driver's license, and auto insurance.

- Work with the Medical Administrator, Deputy Executive Director and Director of Patient Services to provide an on-going, comprehensive integrated self-assessment of the quality and appropriateness of care and services provided.
- Makes recommendations based on findings to correct identified problems and revise hospice policies as necessary.
- Implement and report on activities and mechanisms for monitoring quality of patient care. Identify and assist in resolving problems and make suggestions for improving patient care.
- Coordinate the implementation of the program’s Quality Assessment and Performance Improvement Plan to assess the quality and appropriateness of patient care.
- Coordinate all QAPI audits and studies as indicated in the plan.
- Coordinate and implement problem focused audits, new forms and new employee orientation for clinical staff.
- Review all incidents, concerns and grievances to ensure appropriate follow-upand documentation have occurred.
- Identify patterns and trends and report them to the Medical Administrator and QAPI committee.
- Conduct quarterly QAPI meetings, reports findings and coordinates development of action plans with Director of Patient Services, Deputy Executive Director and Medical Administrator.
- Complete monthly reports to the Medical Administrator, Deputy Executive Director and Director of Patient Services.
- Utilize problem-solving techniques to address areas of non-compliance.
- Maintain current information and requirements related to applicable laws, regulations and standards.
- Assure the program is reviewed on an ongoing basis. Included is an evaluation of all services that were provided and patients/caregivers response to those services. Attention is given to: symptom management, pain control, stress management, continuity of care and inpatient care.
- Maintain continuing education through academic studies, seminars, and workshops
- Attend and participate in in-services, staff meetings and case conferences.
- Complete all documentation in an accurate, legible manner and the timeframe required by Hospice of Valleys' standards and regulations.
- Assist Director of Patient Services with daily office coordination of care, as required.
