Hospice of the Valleys serves those with a life-limiting illness, regardless of illness or age, who live in southwest Riverside County.
Only Hospice of the Valleys provides this unique and comprehensive scope of services:
- In-home hospice services
- Inpatient level of care in collaboration with various facilities located in our service area
- Full-time Medical Directors on staff
- Hospice of the Valleys Grief Support Services counseling and support
- Individualized, interdisciplinary care teams with 24-hour, 7 days-a-week accessibility to highly skilled, dedicated clinical staff
Hospice of the Valleys is a not-for-profit agency, accredited by the Community Health Accreditation Program (CHAP). We are a proud member of the National Hospice and Palliative Care Organization (NHPCO) and the California Hospice Foundation (CHF). We are also affiliated with the National Institute for Jewish Hospice (NIJH).
Referral/Admission Process
We are happy to speak with interested patients, family members or loved ones about Hospice of the Valleys services. If interested, we can contact the patient’s physician for needed information. A member of Hospice of the Valleys interdisciplinary team will then visit the patient where ever they call home, or in the hospital. Hospice services will be discussed with the patient and family and the assessment process will begin to include a plan of care that will address the patient’s unique needs.
