What is Transitions?

Transitions provides emotional support and practical asistance for clients and families coping with life-limiting illness.

Transitions is a community service program offered by HOSPICE OF THE VALLEYS  to provide emotional support, education, and practical assistance to individuals and families facing a life-limiting illness. There is no fee or charge for these services. The program is coordinated by a licensed clinical social worker who meets with the client and family to formulate an individual plan based on their specific needs. The program utilizes caring, trained and dedicated volunteers who offer support, friendship, and respite to the clients and their families.

Many chronically ill and terminally ill individuals desperately need servises but never receive them because their conditions don't meet the strict guidelines of home health care, their conditions are not yet hospice appropriate, or they are not yet ready to select hospice care. What Does Transitions Do?

Transitions provides clients with information and referrals to community services and places volunteers with patients to offer one-on-one support and assistance.

Transitions provides clients with a Coordinator, who is a Medical Social Worker, to assess clients' needs and coordinate clients' care. The Coordinator can answer questions, provide assistance with advanced health care planning, and guide clients to community services like Meals on Wheels, homecare, transportation, and hospice services. The Coordinator stays in contact with clients and their families by telephone or home visits to insure that appropriate resources are provided.

Transitions can assign volunteers to clients to provide emotional support, assistance with transportation, light housekeeping, and breaks for family caregivers.

How Can I Receive Transitions Care?

What do Transitions Volunteers Do?

Volunteers play an important role in fulfilling the mission of Transitions. After successfully completing an intensive training program, Transitions Volunteers provide companionship, assistance with shopping and errands, and transportation to appointments.
