Are you one of the many terminally ill individuals who need additional help but are not yet ready for hospice care? Hospice of the Valleys Transitions Program can help.

Hospice of the Valleys Transitions Program is a free, community service program designed to provide emotional support, education, and practical assistance to individuals and families facing a life-limiting illness. There is no fee or charge for these services.

The program is coordinated by a specially trained hospice professional who meets with the client and family to formulate an individual plan based on their specific needs. The program utilizes caring, trained and dedicated volunteers who offer support, friendship, and respite to the clients and their families.

What Does the HOV Transitions Program Provide?

  • Offers clients Information and referrals to community services and places volunteers with clients for one-on-one support and assistance
  • Assigns a Community Liaison to each client to assess clients' needs and coordinate clients' care. The Community Liaison can answer questions, provide assistance with advanced health care planning, and guide clients to community services like Meals on Wheels, homecare, transportation, and hospice services. The Community Liaison stays in contact with clients and their families by telephone or home visits to insure that needs are being met
  • Transitions can assign volunteers to clients to provide emotional support, assistance with transportation, light housekeeping, and breaks for family caregivers
