Per Diem RN Case Manager

Position Summary

The hospice case manager is responsible for initiating and coordinating the hospice plan of care to each patient he/she has been assigned.  The RN Case Manager works with the attending physician, Hospice Medical Director, and other members of the interdisciplinary Hospice team to provide skilled nursing and palliative care to his/her patients.  The RN Case Manager performs prescribed medical treatments, including pain management and symptom control, conducts assessments and evaluations, and provides education and supportive care to patient and caregiver.

Qualifications

Current RN license within the state of California

Minimum of one-year experience as a professional nurse within the last 3 years

Valid California drivers license and auto insurance
